Parents need to know that The First Omen is not for kids. Not only is the horror and gore intense, but there is also nudity and implied rape.

The First Omen is rated R by the MPA for Violent Content, Disturbing Images, and Brief Graphic Nudity which means some content is not suitable for kids under 17 years old.
Language
There is some profanity including the uses of f*ck and f*cking and words of Deity.
Violence
Violence is quite graphic including nuns being tied and hooded so demons could rape them. There is a lot of blood and gore with use of weapons and exposed body parts. A childbirth scene includes a lot of blood and is intense. A nun sets herself on fire. There are a lot of jump scares. Characters are seen in peril, tortured, and are killed.
Mature Content
There is some genital nudity in The First Omen. A demonic claw comes out of a pregnant woman’s vagina.
The First Omen is very dark and has themes of evil and the devil.
Is The First Omen Appropriate for Kids Under 17?
You can expect lots of horror, gore, violence, some nudity, and mature, dark themes in The First Omen. To sum it up, it is not appropriate for kids under 16 years old. The First Omen is rated R for many reasons and at times feel like it could be rated NC-17.
Nell Tiger Free plays Margaret, the American girl who comes to Rome at the Vizzardeli Orphanage and take her vows as a nun. She nails all the emotions as the horrors begin to unleashe in the orphanage.
Some of the scenes are hard to stomach and the film can get chaotic, but horror fans may be pleasantly surprised with this prequel. If you’re super religious, maybe skip this one, because things can get super unholy. Religious fanaticism seems to be a theme in 2024 with films.
